Seasonal Flu Vaccines 2025-2026

Vaccinations are usually billed with two CPT® codes: one for the vaccine and one for the administration of the vaccine. If the practice did not pay for the vaccine, only the CPT® code for the administration of the vaccine is reported. When the vaccines are supplied by a Vaccine for Children Program (VFC), how the administration of the vaccine is reported will vary by State Medicaid carrier.  

All vaccines are reported with diagnosis code Z23 (Encounter for immunization).  

If the sole reason for the visit is to receive a vaccine, an office visit should not be reported.  

Many flat/case rate plans exclude routine services including vaccinations. In that instance, patients should pay cash at the time of service.  

Flu vaccine codes change yearly depending on what will be effective for the current flu season.  

The codes listed in the Administration Code column below are the most used administration codes. There are instances when the administration code may be different:  

• Medicare: The administration is reported with HCPCS G0008, (Administration of influenza virus vaccine) for Medicare beneficiaries.  
• Pediatrics: If the visit includes counseling for patients up to and including age 18, see CPT® code 90460.  
• Multiple: If multiple vaccines are administered at the same visit, the subsequent vaccines are reported with an add-on code (e.g., 90472).
