To obtain medical records for any reason, please send a HIPAA compliant release form signed by the patient to records@experityhealth.com or fax your subpoena to 779-666-8065.
We do not bill patients or insurance companies directly, or own the images sent to us for interpretation. You must contact the facility where the exam was performed if you require billing information or a disc with images. There is no separate bill from Experity Teleradiology, and we are unable to release images.
