During our April webinar series, product owner Sara Wirth demonstrated how to use the newly released chronic problem management (CPM) tab inside our EMR. Sara explained how to access previous HPI, assessments, and plans, follow-up on chronic problems, and HPI coding score calculation.

This new tab is available to all PV users and is the first step in a line of enhancements that will continue to support primary care. Within the HPI tab, users can easily see the patient’s previous visits, conveniently located on the right-hand side. The visits can be filtered by date, chief complaint or chronic problem, with users being able to see notes and details about those visits.

To begin treating a chronic problem that a patient presents, users can view the active chronic problems for the patient in the CPM section and choose which problem they wish to follow-up on. Users can insert details about the visit into the “current status” field manually or use the copy button to duplicate the last status into the current status field if nothing has changed from the previous visit.

But what if a patient presents with a chronic problem and also needs additional support with an episodic issue? Users can easily switch to the chief complaint tab once they have documented the chronic problem and begin documenting acute problems as needed.
